TITLE: Performance Evaluations Career Development Plan

POLICY

The Fifth Judicial District Department of Correctional Services will complete written performance evaluations on all employees. Employees will be evaluated and scored objectively using specific job classification criteria and scoring guide.

New employees to the District will receive written performance evaluations during their six (6) month probationary period at two (2) months and five (5) months. Upon successful completion of their probationary period employees will be evaluated on or near their designated annual review date thereafter.

Promoted employees will be evaluated five (5) months after their promotion date and will be evaluated on or near their designated annual review date thereafter.

Any employee can be evaluated at any time if it is determined to be appropriate by the designated supervisor and their assistant director.

PROCEDURE

  1. The employees will be evaluated by their immediate supervisor.
  2. Supervisors will review and discuss the evaluation with the employee.
  3. The Supervisor will also discuss career goals and achievements and accomplishments.
  4. The Supervisor and the employee will sign the evaluation.
    1. The original evaluation goes to the Personnel Technician to be kept in the personnel file.
    2. The Supervisor keeps a copy of the evaluation for his/her employee file.
    3. The employee keeps a copy of the evaluation.
